Alameda County Behavioral Health Care Services – TAY System of Care

Mission

The Alameda County Transition Age Youth (TAY) System of Care exists to improve the services and outcomes for youth, age sixteen through twenty-four who are experiencing severe mental illness, in making successful transitions towards self-sufficiency and independent living.

Services

Programs for TAY based on Wellness, Recovery, and Resiliency create real hope that a person can recover from mental illness and go on to live happy, productive lives. We primarily serve youth who as a result of their mental disorder have substantial impairment in self-care, school functioning, family relationships, or in their ability to function in the community.


A Transition Age Youth Advisory Board was formed in partnership with the Pool of Consumer Champions to ensure a continuous youth voice in the planning of new programs and services. They meet monthly to contribute to how TAY mental health services are designed and delivered in Alameda County, to acquire valuable experience and leadership skills, and to have access to professional conferences, seminars and work opportunities related to mental health.


Transition to Independence Program (TIP) Services include intensive case management, medication support, counseling.


Supported Housing for Transition Age Youth (STAY) Program Services include case management, medication support, counseling, housing, and 24-hour services.


STARS Community Services Services include case management, medication support, counseling, and peer groups.


Youth UpRising Mental health services at a youth development center for young people without health insurance. YU offers activities for youth in media, dance, computer, and GED studies in a safe place for all youth.

About Alameda County Behavioral Health Care Services – TAY System of Care

The TAY System of Care bridges adult services and children’s services with new programming specifically designed for young people. It is a collaborative effort between Alameda County Behavioral Health Care Services, transition age youth from Alameda County, their families, and youth service providers, including those in vocational rehabilitation, education, foster care, ILSP, juvenile justice, and probation systems.
